Methods: This study is included in an observational study with a cross sectional study design involving 75 female students selected purposively then measured nutritional status and interviewed directly with the Menstrual History questionnaire, Pittsburgh Sleep Quality Index (PSQI), and Depression Anxiety and Stress Scales (DASS 42), and International Physical Activity Questionnaire-Short Form (IPAQ-SF). Statistical analysis was performed with the Chi-Square Test. Results: There was a significant relationship (P=0.011) between physical activity and the incidence of primary dysmenorrhea but there was no relationship between menstrual cycle (P=0.775), sleep quality (P=0.486), stress level (P=0.918), BMI-for-age nutritional status (P=0.819), and %MUAC (P=0.721) with the incidence of primary dysmenorrhea. Conclusion: Physical activity is significantly associated with the incidence of primary dysmenorrhoea in female students. The management of boarding school needs to promote routine physical activity into the school program to prevent primary dysmenorrhea in female students. Penelitian ini bertujuan untuk menganalisis hubungan siklus menstruasi, kualitas tidur, tingkat stres, aktivitas fisik dan status gizi dengan kejadian dismenorea primer pada santriwati di Pondok Pesantren Roudhotul Qur’an, Depok, Jawa Barat. Metode: Penelitian ini termasuk ke dalam studi observasional dengan desain studi cross sectional melibatkan 75 santriwati dipilih secara purposive kemudian diukur status gizi dan diwawancarai secara langsung dengan kuesioner Menstrual History, Pittsburgh Sleep Quality Index (PSQI), dan Depression Anxiety and Stress Scales (DASS 42), dan International Physical Activity Questionnaire-Short Form (IPAQ-SF). Analisis statistik dilakukan dengan Uji Chi-Square. Hasil: Terdapat hubungan signifikan (P=0,011) antara aktivitas fisik dengan kejadian dismenorea primer namun tidak terdapat hubungan antara siklus menstruasi (P=0,775), kualitas tidur (P=0,486), tingkat stress (P=0,918), IMT/U (P=0,819), dan %LiLA (P=0,721) dengan kejadian dismenorea primer. Simpulan: Aktivitas fisik berhubungan nyata dengan kejadian dismenorea primer pada santriwati.
