ANALISIS PENYEBAB DEFECT DENGAN MENGGUNAKAN METODE FAILURE MODE EFFECTS AND ANALYSIS DAN FAULT TREE ANALYSIS PADA ASSEMBLY AREA PT EBAKO NUSANTARA

Abstract

PT. Ebako Nusantara merupakan salah satu perusahaan yang bergerak dalam bidang furniture yang berlokasi di Kawasan Industri Terboyo, Semarang. Perusahaan ini bergerak dalam pembuatan Artefact, Bed, Cabinet, Cocktail TABLE, Table, Barstool, Bench and Chair. Berdasarkan data hasil laporan bulanan terdapat 11 jenis cacat. Jenis cacat yang paling banyak ditemui adalah masalah surface problem, yaitu sebesar 37,88 %. Upaya yang dilakukan untuk pengendalian produk gagal tersebut adalah dengan menggunakan metode Failure Mode and Effect Analysis (FMEA) dan metode Fault Tree Analysis (FTA) untuk mengidentifikasi dan menganalisa kegagalan yang terjadi. Tujuan dari penggunaan FMEA adalah untuk mengetahui moda kegagalan mana yang memiliki nilai RPN paling tinggi dengan cara mengalikan tingkat keparahan, kejadian, dan deteksi. Selanjutnya moda kegagalan dengan nilai RPN diatas 100 dijadikan sebagai top level event pada diagram FTA. Metode FTA digunakan untuk mengetahui akar penyebab kegagalan yang terjadi. Pada PT. Ebako Nusantara moda kegagalan yang nilai RPNnya diatas 100 ada yaitu surface problem.Penyebab kegagalan potensialnya adalah karena kondisi kayu yang kurang baik, pemotongan kayu tidak rata dan kualitas amplas tidak baik

ABSTRACT

PT. Ebako Nusantara is one of the companies engaged in the field of furniture located in the industrial area Terboyo, Semarang. The company is engaged in the manufacture of Artefact, Bed, Cabinet, TABLE, Cocktail Table, Barstool, Bench and Chair. Based on the results of the monthly reports there are 11 types of defects. Types of disabilities the most ubiquitous is the surface problem, namely the problem of 37.88%. Efforts are being made to control the failed product is by using the method of Failure Mode and Effect Analysis (FMEA) and the method of Fault Tree Analysis (FTA) to identify and analyse the failure happened. The purpose of the use of FMEA failure mode is to find out which ones have the highest RPN value by way of multiplying the severity, occurrence, and detection. Next mode of failure with the value of the RPN above 100 as a top tier event on the diagram of the FTA. The FTA method is used to find out the root cause of failures that have occurred. At PT. Ebako Nusantara is the failure mode of the RPNnya value above 100 there are i.e. surface problem. The cause of the failure is because of the tremendous potential that wood conditions are unfavourable, the cutting of the wood and the quality of the sandpaper is not good
