East Kalimantan Fertilizer, namely HTS Effluent, a leakage was identified in the seat flange leading to the tube bundle to shell, which could pose a hazardous situation, necessitating a factory shutdown for safety. Through Mean Time Between Failure (MTBF) and Failure Rate analysis, HTS Effluent is identified as the main cause of failure with an MTBF of 1,385 years and a failure rate of 0.722 per year. The Failure Mode and Effect Analysis (FMEA) method was employed to assess the criticality level of the HTS Effluent. The item showed the highest Risk Priority Number (RPN) at 180. Subsequent analysis using Fault Tree Analysis (FTA) revealed that the Turn Around (TA) interval for the Ammonia area is below the target of 2 years. Further improvements are required to achieve the target TA interval and minimize the impact of failures on critical items.
