Pada salah satu item Pabrik 5 PT. Pupuk Kalimantan Timur yaitu HTS Effluent, ditemukan bahwa
kebocoran pada seat flange menuju tube bundle to shell dapat menyebabkan situasi berbahaya,
memerlukan shutdown pabrik demi keamanan. Melalui analisis Mean Time Between Failure (MTBF) dan
Failure Rate, item HTS Effluent diidentifikasi sebagai penyebab utama kegagalan dengan MTBF sebesar
1,385 tahun dan failure rate sebesar 0,722 tahun. Metode Failure Mode and Effect Analysis (FMEA)
digunakan untuk menilai tingkat kekritisan item. Item HTS Effluent menunjukkan nilai Risk Priority
Number (RPN) tertinggi, yakni 180. Analisis setelah FMEA dilakukan dengan metode Fault Tree Analysis
(FTA), menunjukkan bahwa interval Turn Around (TA) untuk area Ammonia kurang dari target 2 tahun.
Perbaikan lebih lanjut diperlukan untuk mencapai target interval TA dan meminimalisir dampak kegagalan
pada item kritis. Studi ini memberikan kontribusi pada pemahaman kritis terhadap permasalahan unit
Ammonia dan merumuskan langkah-langkah perbaikan yang diperlukan.

Abstract

[Title: TURNAROUND INTERVAL PLANNING AND CONTROL THROUGH THE DEVELOPMENT
OF FMEA AND FTA METHODS BASED ON CRITICAL ITEMS IN PLANT 5
(Case Study: PT East Kalimantan Fertilizer)] Air On one of the items in Plant 5 of PT. East Kalimantan
Fertilizer, namely HTS Effluent, a leakage was identified in the seat flange leading to the tube bundle to
shell, which could pose a hazardous situation, necessitating a factory shutdown for safety. Through Mean
Time Between Failure (MTBF) and Failure Rate analysis, HTS Effluent is identified as the main cause of
failure with an MTBF of 1,385 years and a failure rate of 0.722 per year. The Failure Mode and Effect
Analysis (FMEA) method was employed to assess the criticality level of the HTS Effluent. The item showed
the highest Risk Priority Number (RPN) at 180. Subsequent analysis using Fault Tree Analysis (FTA)
revealed that the Turn Around (TA) interval for the Ammonia area is below the target of 2 years. Further
improvements are required to achieve the target TA interval and minimize the impact of failures on critical
items. This study contributes to a critical understanding of the Ammonia unit issues and formulates
necessary improvement steps.
