PT Pertamina (Persero) melakukan proses pendistribusian melalui berbagai opsi transportasi. Untuk proses pendistribusian jalur darat, Pertamina menggunakan dua opsi kendaraan, yaitu kereta api dan mobil tangki. Pendistribusian dengan mobil memiliki tingkat risiko yang lebih tinggi dibandingkan dengan menggunakan kereta api. Dengan target Pertamina yaitu zero accident dalam pelaksanaan K3 perlu dilakukan analisis kualitas terkait fleet safety management mobil tangki untuk mengetahui tingkat kualitas suatu hal secara keseluruhan maupun hanya bagian-bagiannya. Analisis kualitas fleet safety management menggunakan pendekatan Lean Six Sigma melalui pendekatan Define, Measure, Analyze, Improve, and Control (DMAIC) dan perhitungan Defect Per Million Opportunity (DPMO). Analisis lebih dalam mengenai faktor-faktor penyebab kecelakaan transportasi mobil tangki baik secara internal maupun eksternal menggunakan tools berupa diagram pareto, Systematic Cause Analysis Technique (SCAT), dan fishbone diagram kemudian rekomendasi perbaikan untuk mengurangi kecelakaan mobil tangki.

Abstract [Quality Analysis Of Tank Car Fleet Safety Management Using Lean Six Sigma Approach And Systematic Cause Analysis Technique (Case Study: PT PERTAMINA MOR III] PT Pertamina (Persero) carries out the distribution process through various transportation options. For the land distribution process, Pertamina uses two vehicle options, namely trains and tanker cars. Distribution by car has a higher level of risk than using trains. With Pertamina's target of zero accident in the implementation of K3 it is necessary to conduct a quality analysis related to fleet safety management of tank cars to determine the level of quality of a thing as a whole or only its parts. Fleet safety management quality analysis uses the Lean Six Sigma approach through the Define, Measure, Analyze, Improve, and Control (DMAIC) approach and the calculation of Defects Per Million Opportunity (DPMO). In-depth analysis of the factors causing tank car transportation accidents both internally and externally using tools in the form of Pareto diagrams, Systematic Cause Analysis Techniques (SCAT), and fishbone diagrams then recommendations for improvements to reduce tank car accidents.
