Malnutrition surveillance can't be separated from ethical issues. So, the research is needed to analyze the ethics of implementing malnutrition surveillance at the Puskesmas Sragi I and Puskesmas Sragi II, Pekalongan Regency based on deontological theory. This is a descriptive research with a qualitative study design. Data collection was carried out through semi-structured interviews with 15 respondents selected by purposive sampling technique. Data analysis presented descriptively with a theoretical basis in accordance with the findings in the research. This research showed that the implementation of malnutrition surveillance is carried out by nutrition practitioners who have an educational background in the field of nutrition. The implementation of malnutrition surveillance at the Puskesmas Sragi I and Puskesmas Sragi II has been carried out well as evidenced by a structured nutritional surveillance recording and reporting system. The boundaries of the ethical principles use deontological ethical principles so that malnutrition surveillance are still carried out because they don't pose a potential risk of harm to patients and provide more benefits, but patient autonomy is still prioritized. Ethical challenges often arise because personal information affects all aspects of life, especially when stigma and discrimination accompany an individual's condition.
