Analisis Kecelakan Kerja dengan Menggunakan Metode Human Factor Analysis and Classification System (HFACS) dan 5WHYS di Divisi Stamping PT.Mekar Armada Jaya

Abstract

PT. Mekar Armada Jaya biasa disebut New Armada dibagi menjadi dua divisi produksi yaitu Divisi karoseri dan Divisi Stamping & Tools. Pada Divisi Stamping & Tools tahun 2010 sampai dengan 2014, terjadi 69 kecelakaan kerja dimana kejadian tersebut dibagi lagi menjadi tiga kategori dalam level rendah, sedang, dan besar. Dari hasil rekap kecelakaan kerja banyaknya terjadi kecelakaan pada mesin press yaitu sebanyak 17 kejadian pada level sedang.

Penelitian pada PT. Mekar Armada Jaya ini bertujuan untuk melakukan identifikasi masalah yang terjadi pada level sedang, setelah dilakukan identifikasi maka kejadian yang terbesar terdapat pada mesin press. Setelah mengetahui kecelakaan kerja terbesar terdapat pada mesin press maka dilakukan analisis dengan menggunakan metode HFACS dan 5whys yang bertujuan untuk mendapatkan identifikasi penyebab terjadinya kecelakaan kerja.

Hasil dari analisis menggunakan metode HFACS maka didapat nilai tertinggi terdapat pada sub level comunication & coordination pada level 2 dan decision error pada level 1 dengan nilai odds ratio sebesar 14 dan convidence interval sebesar 0,451 – 434,411. Maka perlu perbaikan pada lingkungan kerja, memberikan peraturan yang memaksa operator untuk bertindak aman sebelum melakukan pekerjaan, pengawas harus bertindak tegas kepada operator yang melakukan kesalahan, pengawas bertanggung jawab atas operator dengan cara mengetahui SOP penggunaan mesin dan memberikan pengawasan secara teratur.

                PT . Mekar Armada Jaya as known as New Armada divided into two production division, named Car Body division and Stamping & Tools division. In Stamping & Tools Division from 2010 to 2014, occurred 69 work accident where those cases are divided into three categories there are low, medium, and high level. From the results recap many working accidents mostly happen in the machine press accident occurred as 17 cases in medium levels.

                The resesearch at PT . Mekar Armada Jaya aims to identify the problems that occur at medium level , after the identification of the biggest incident occur in the press machine. After knowing  that the highest number of working accident in the medium level occur in the press machine, therefore this analysis performed by using HFACS and 5whys methods which aim to get identify the causes of working accidents.

                    Results of the analysis is using HFACS method then the gained highest score contained in the sub-level of comunication and coordination at the 2nd level and the decision error at the 1st level with the odds ratio of 14 and convidence interval of 0,451-434,411. So the working environment need to be fixed, giving rules which force the operator to act safely before doing the job, the supervisor must be firm to the operator who do mistake, the supervisor must responsible to the operator by understanding the SOP of the machines usage and give supervision in order.
