ANALISIS TRAFFIC ACCIDENT DI INDUSTRI TAMBANG BATU BARA DENGAN METODE HUMAN FACTOR ANALYSIS AND CLASSIFICATION SYSTEM-MINING INDUSTRY (HFACS-MI) (Studi Kasus di PT X Rantau Distrik KCMB)

Abstract

PT X Rantau Distrik KCMB adalah Perusahaan penambangan batubara mulai dari tahap eksplorasi, eksploitasi, transportasi hingga pelaksanaan reklamasi tambang. Kegiatan tambang tersebut memiliki potensi bahaya dan resiko terjadinya kecelakaan dalam setiap proses penambangan. Kasus kecelakaan kerja dibagi dalam 2 bagian yaitu major loss dan minor incident,minor incident dibagi menjadi 3 bagian yaitu traffic accident, tale getting, dan minor injury. Traffic accident adalah kecelakaan kerja  yang terjadi dijalur tambang atau dijalur hauling. Traffic accident merupakan kejadian yang paling sering terjadi dan sering menjadi penyebab pengeluaran perusahaan yang paling tinggi. Metode HFACS-MI bertujuan untuk mencari basic event atau akar masalah yang terjadi dalam traffic accident. HFACS-MI memiliki 5 level yang menjadi faktor utama dalam investigasi kecelakaan. Hasil dari investigasi didapatkan bahwa unsafe act merupakan penyebab utama terjadinya traffic accident yang memiliki nilai faktor kejadian sebanyak 29 faktor kejadian dari 29 traffic accident.

ABSTRACT

PT. X Rantau KCMB Distric the company of the coal mining starting from exploration,exploitation,transportation to execution of mine reclamation. The mining activity has petential danger and risk of accident in every process of mining. Work accident cases are divided into two parts are major loss and minor accident, a minor accident is divided into three parts namely traffic accident,tale getiing, and a minor injury. Traffic accident was a work accident that happenened had we had a minr or hauling. Traffic accident is an occurrence that most frequently occur and are often the cause of the company,s expenses. HFACS-MI has five levels that became the major factor in the investigation of accidents. The result of investigation found that an unsafe act are the main cause of occurrence of traffic accident factor that has value by as much as 29 incidence factor of 29 traffic accident.
