Penilaian risiko dilakukan menggunakan indeks likelihood dan severity serta risk rating matrix berdasarkan standar AS/NZS 4630:2004. Area dengan tingkat risiko ekstrem tertinggi adalah ruang penyimpanan klorin (80%), diikuti oleh IPA IV (14,29%). Rekomendasi pengendalian disusun berdasarkan hierarki pengendalian risiko, dengan penekanan pada perbaikan organisasi, penambahan alat pelindung diri, dan peningkatan kesadaran K3 di kalangan pekerja. Ditemukan bahwa kesadaran K3 di perusahaan masih rendah, ditandai dengan minimnya penggunaan APD dan tidak adanya divisi khusus K3. Oleh karena itu, perbaikan berkelanjutan sangat diperlukan untuk meningkatkan keselamatan kerja di lingkungan PDAM. The results show that there are 14 risks in the IPA IV area, 5 risks in the chlorine storage room, and 6 risks in the operator room. Risk assessment was conducted using a likelihood and severity index and evaluated with a risk rating matrix based on AS/NZS 4630:2004. The chlorine storage room was found to have the highest level of extreme risk (80%), followed by the IPA IV area (14.29%). Risk control recommendations were developed based on the hierarchy of controls, emphasizing organizational improvements, use of personal protective equipment (PPE), and increasing awareness of occupational safety and health (OSH). The study also found that OSH awareness among workers is still low, as indicated by the lack of PPE use and absence of a dedicated OSH division. Continuous improvement is needed to enhance workplace safety in PDAM’s operations.
