Abstract
The Antenatal Care (ANC) program is a key public health policy aimed at reducing the Maternal Mortality Rate (MMR). Bogor Regency was selected for this study due to its persistently high MMR despite achieving administrative ANC coverage. This study analyzes the implementation of the ANC program and factors influencing the utilization of ANC services in reducing maternal mortality. A qualitative descriptive approach was employed using in-depth interviews, observations, and document analysis involving health authorities, health facilities, health workers, community cadres, traditional birth attendants, pregnant women, and postpartum mothers. Policy implementation was analyzed using Riant Nugroho’s policy implementation accuracy framework, supported by Andersen’s Behavioral Model. The findings indicate that while ANC implementation aligns with existing regulations, its effectiveness remains limited due to resource constraints, discontinuity of ANC visits up to K6, behavioral factors, and insufficient family support. Strengthening human resources, promoting family-based education, and integrating behavioral approaches are essential to improve sustainable ANC utilization and maternal health outcomes
